Lunch & Learn: Advance Care Planning

Advance Care Planning helps you make your health care wishes known now, so your loved ones and health care team can honor your choices if you are unable to speak for yourself. This seminar will help you: • Think about what is most important to you for your future health care • Choose a trusted person to make health care decisions for you if you cannot • Discuss your wishes with family, caregivers and health care providers • Complete important documents such as advance directives or health care power of attorney forms • Document your preferences for treatments and end-of-life care • Review and update your wishes as your health or circumstances change • Make sure your health care team has access to your documented wishes • Help reduce stress and uncertainty for loved ones during difficult times • Ensure care aligns with your values, goals and personal preferences Presenter: Desirae Toomey All are welcome.
